Frequently Asked Questions

  • A hybrid physical therapy practice accepts both insured patients (when their plan is covered) and cash-paying patients. This flexibility helps people in areas with limited access to obtain timely care. Hybrid clinics often provide additional options—such as maintenance visits, telehealth follow-ups, or dry needling—that traditional clinics may not offer, allowing them to better match services to individual patients’ needs.

  • In Ohio you can usually start physical therapy without a doctor’s prescription. As a “direct access” state, you may schedule an evaluation and begin treatment for injury care, prevention, wellness, or performance without first seeing a physician.

    Note: Insurers often require a physician-signed plan of care before they’ll pay. Medicare allows you to start therapy but needs physician certification for coverage. If your therapist thinks you need medical evaluation or aren’t improving, we’ll refer you to the right provider.

  • We put each patient first with personalized, one-on-one care designed to accelerate recovery and enhance long-term outcomes. We value every person who chooses our clinic and warmly welcome them into our supportive care community. In addition, we provide several specialized services that many physical therapy clinics don’t often highlight, including dry needling, ongoing maintenance visits, comprehensive wellness screenings, and infrared light therapy + NORMATEC recovery sessions.

  • We treat patients across nearly every age group, generally beginning around 7 years old (and occasionally younger depending on the specific condition) and extending well into the 99+ range. We do not routinely care for most infants and newborns because certain complex pediatric issues are best managed by specialists at local hospitals and clinics; in those cases we provide referrals and coordinate care as needed.

  • We treat a wide range of conditions, focusing primarily on musculoskeletal issues, post-operative rehabilitation (including total knee, hip, and shoulder procedures), as well as neurologic and cardiopulmonary problems, among others. Dr. Costa also provides specialized care for runners, cyclists, triathletes, and MMA athletes. If you have a less common or complex condition, please call our clinic to discuss your specific needs.

  • Most sessions are one hour in length, though the exact duration may vary according to individual needs. We do kindly ask that patients arrive 5–10 minutes early to allow time for any additional paperwork or check-in procedures.

    For Dry Needling Sessions or Infrared Red Light + NORMATEC Sessions, they are 30 minutes in length.

  • Most forms of payment are accepted, including cash, check, and the majority of major debit and credit cards. We also accept FSA and HSA cards that are issued as VISA or Mastercard to help offset the cost of services.

  • We can most often get patients scheduled and seen within 48–72 hours.

  • We proudly offer weekend hours, unlike most clinics in Ohio. We have appointment availability on both Saturday and Sunday. If Saturday fills up, we allow appointments to carry over into Sunday to ensure patients can still be seen. We believe health comes first for every individual, and care shouldn’t be restricted on the weekends—because life doesn’t stop when the calendar changes.

  • If your insurance isn’t listed, please give our clinic a call and we’ll be happy to help. The insurances that are listed are ones we have contracts with and are considered “in‑network,” but there are many cases in which insurers we don’t have a contract with still offer “out‑of‑network” coverage that provides very similar copays and deductibles, and we may be able to work with those plans. We also work with a third‑party billing team whom we consult to help decipher what out‑of‑network benefits may be accepted and to clarify potential costs for your care.

    If your insurance isn’t accepted, we provide affordable self-pay rates to ensure you can still receive the care you need.

  • Our clinic requires at least 24 hours’ notice for appointment cancellations, and our cancellation policy is as follows: two or more cancellations or no-shows without the required 24-hour notice may result in a $35 fee billed directly to the patient. That said, we understand that unexpected events and illnesses happen. To help accommodate these situations and avoid interruptions in your care, we can often convert an in-person visit to a telehealth appointment, ensuring your treatment progress continues even when you cannot come into the clinic.
