  • CEO/Chief Physical Therapist

    Dr. Costa is a native of the tri-county area. At a young age, he competed in track & field and cross-country, but unfortunately had a career filled with injuries. With the help of a few medical professionals, he had success on the local scene of running. Once entering college, he took inspiration from those who had assisted him, and decided that he wanted to revolutionize the world of healthcare and give individuals the care that they deserve. Prior to attaining his doctorate, he worked for roughly 4 years in the outpatient orthopedic setting as a physical therapist assistant. In 2023, he capped his academic career by attaining his DPT from Youngstown State University.

    Dr. Costa has experience in various settings in the physical therapy field, including: outpatient orthopedics, skilled nursing, home health, hospital-based care (ICU specific), remote therapy, & injury prevention & wellness.

  • Administrator

    Jade is a Certified Occupational Therapist Assistant by trade and has a passion to assist those in need. Jade is local to the tri-county area where during her youth she stayed very active with soccer, volleyball, & track & field. Jade began her college career at Youngstown State University prior to attending Penn State University, where she received her degree in occupational science. Jade has experience in outpatient orthopedics & skilled nursing.

    During 2021, Jade found a passion for mixed martial arts and has trained in Brazilian Jiu Jitsu & upright combat since.

    Costa Rehab & Wellness is pleased to have Jade as an administrator, overseeing operation management & patient satisfaction.
